Liberty Middle School Activity Registration

 ACTIVITY REGISTRATION IS NOW OPEN!

All 7th & 8th-grade students participating in an athletic activity during the academic school year are required to upload a valid Sports Physical as a part of their registration.  The physical must be dated  on or after April 15 to be valid for participating in the following school year. Once submitted, the physical will be valid for the entirety of the school year.   

*6th graders are required to submit a sports physical if they intend to participate in the spring session of Form & Fitness.  

If your physician does not provide a copy of the sports physical, a physical form can be downloaded here.  *Only page 6 of the physical form is required in order to participate in middle school activities. To be considered valid, the physical must include a physician signature, as well as a parent/guardian signature in the "Permission for Medical Treatment" section.  

Activity Registration:

1.  You will need your district assigned, Student ID Number (If you are new to the district, you will receive your student's ID number within a week of registering for school).  
2. Complete Online Registration using the options below.
3. Upload a valid sports physical as a part of registration. 
4. Submit activity fee through MySchoolBucks.
5. Record email & password for future activities registrations.
